The Long Term Support Unit provides choices, resources, and services to elderly individuals, to persons with physical or developmental disabilities and to individuals with mental health and alcohol/other drug abuse needs to enhance independence, stability, dignity, and self-worth.

Long Term Support offers service options for people of all ages that have a chronic disability attributable to mental or physical disability or their normal aging process.  These services are intended to allow people to remain in the community and stay out of nursing homes or other institutional settings unless absolutely needed.  Staff will determine eligibility and support needs through an individualized assessment process.  Following that, a service plan will be jointly developed by you and the case manager or service provider to meet your needs.
